  • Before you get started

    Please ensure you have read the manual and followed all relevant troubleshooting steps. Once you submit this form a copy will be emailed to you for your records including a reference number which can be used in all future communication.

    Once your claim has been accepted you will be contacted by a service technician to book a time to attend the site to repair the ceiling fan within business hours Monday to Friday. Please allow up to 7 days from when your claim is accepted.

    To complete this form you will require the following;

    1. A copy of your Purchasing Recept  (in either PDF, JPG, GIF format). 
    2. A copy of your Proof of Licensed Installation – this can be an invoice or letter on the companies letterhead (in either PDF, JPG, GIF format)
    3. (Optional)  Photos or video's showing the fault – that can assist with identifying the fault. Thank you in advance, if you have any questions please do not hesitate to contact us.
